Evaluate LeMill's accessibility

Description

The LeMill system should be evaluated with regard to its accessibility. See ​http://www.w3.org/TR/WAI-WEBCONTENT/ for level A, AA, and AAA criteria.

Some of these can be checked with a validator, but most criteria need to be manually and visually validated.

Report findings in the wiki page Accessibility.

I've added some info to the wiki page. The list only contains the problems which are needed to fix to reach the minimum A rating. There are other issues as well, like not all form controls are inside a <form></form> tag pair (excersize template mostly). But these are required to achieve higher ratings.
